Please complete the enrolment form to enrol your child. The question regarding your child's medical conditions/allergies, is to allow GDA to keep your child safe during our sessions. Along with any other medical conditions, issues which mean your child needs to bring certain items to our classes, such as inhalers, insulin, or EpiPens etc, should be brought to our attention.
